1. **** $30 deposit required, non-refundable if cancellation is within 7 days of appointment. If cancellation is more than 7 days before appointment, refundable

2. A rabies vaccine is required at the time of surgery if your pet is not up-to-date. We offer the vaccine discounted at $10 at the time of surgery. If your pet is up-to-date we must have proof in the form of a physical rabies certificate signed by a veterinarian within the last year, this can be uploaded when making the appointment or emailed to parnter@4directionsvet.org.

3. HUTINGTON COUNTY RESIDENTS: Your total surgery cost is $30 and includes a rabies vaccine. This price is made possible by grants provided by Hutington County and the Hutington County Humane Society. This is valid only for Hutington County, IN residents and you will need to have a valid license verifying your residency. Any additional costs are at the owner's expense at the time of pickup.

4. FURRY FIX DAYS (Wabash)- Any days advertised for Furry Fix the total surgery cost is $55 dollars for ALL PUBLIC. You will pay a $30 deposit and the rest will be due at the pickup along with the cost of any other additional services. This price is made possible by donations and grants offered through Furry Fix.

5. VOUCHERS for VanWert, OH: Please bring your vouchers at pickup or check-in, we do collect them, thank you!

5. Any animal in for surgery gets flea combed and checked from head to toe. If we find live fleas, we administer a capstar pill for a cost of $8. This kills adult fleas for 24 hours, it allows us to keep the surgery site on your pet flea free, and to avoid fleas jumping around the mobile unit.

6. Once you sign up a consent form will be emailed to you, please read it, check and initial all boxes, sign it, and submit it. This makes check-in much easier.
